Gross pay across pay periods

Weekly gross is your rate times regular hours, plus 1.5× the rate for any overtime. From there, other pay periods are just multiples: biweekly is two weeks, annual is 52 weeks, and monthly is the annual figure divided by 12 (not four weeks — months are slightly longer).

Why isn't monthly just weekly × 4?

A month averages about 4.33 weeks. Using weekly × 52 ÷ 12 spreads the year correctly, so you don't undercount two to three paychecks a year.
